Laser-assisted liposuction has been known for over a decade, but while the old lasers aimed at destroying the fat cell, the new trends are just the opposite – they aim at preserving the fat cell (which can be transferred to another location if necessary), breaking down the connective tissue binds and linings around the fat cells, as well as tightening the skin.

The term lipofilling (from lipo – adipose tissue and filling) has been introduced in contemporary plastic surgery by its discoverer – Dr. Coleman from New York, whose lectures and training Dr. Dinev MD had the honor of attending while specializing in Paris. The term “fat transfer” is used also for lipofilling.

Fat transfer is a special type of liposuction that removes the fat cells from that part of the body where they are in excess. They are processed so as to be separated from serum and electrolytes and are re-injected, during the same session, into a new location of the body where they are needed: to increase breasts, buttocks, lips or cheekbones. One month after surgery, half of the injected cells die and are metabolised. The remaining cells are fed into the new location and remain alive there. Dying cells stimulate the growth of stem cells from the fat tissue (they have increasingly been used by regenerative medicine) and thus have a beneficial effect on the skin under which they are injected. It rejuvenates, becomes more fresh and elastic, and wrinkles are smoothed.

Through the lipofilling method we can modify several different parts of the body – to reduce some of them (abdomen, love handles, waist, thighs, etc.) and increase others instead (breasts, buttocks, lips, cheekbones, etc.). Breast reduction surgery is similar to the breast lifting surgery. The difference is that in addition to cutting a skin fold, we also cut of a part of the gland and the underlying adipose tissue. In cases when the breasts are so big that they cause back pain, the breast reduction surgery relieves the back and the pains disappear. The scars that remain are anchor shaped and, if necessary, can be corrected through lasers, injections and creams. There is no particular pain in the postoperative period. Breast lifting

There are different methods for breast lifting depending on the degree of sagging.

Breasts can be lifted by placing breast implants, through lipofilling (fat transfer) or cutting the skin. Cutting can only be around the areola or cutting out a larger amount of skin in the lower part of the breast, leaving an anchor-shaped scar. We use the Pitanguy’s technique, modified at St. Louis – Paris Hospital. The scars are thin lines, usually hidden in the breast creases or around the areola. In this method there is no severe pain in the postoperative period. The operation lasts for about 3 hours in general anaesthesia. The stay at the clinic is one night. On the second day the drains are removed and a bra is put which has to be worn for 1 month.

If necessary, the scars can be further corrected surgically or through laser procedures.

36Under this term we understand tightening of the abdominal skin and abdominal muscles of women after childbirth, often accompanied by liposuction of adjacent areas. The scar is in the lower abdomen over the pubis and is hidden in the underwear. The operation is under full anaesthetics, and involves pre-shaving the pubic area, stay at clinic for at least one night, and wearing tight body in the following month. A few days after the operation, abdominal pain is possible due to the stitches on the abdominal muscles, which are coped with conventional pain relievers. Hard physical activity can be resumed in a minimum one month after the intervention.

What the patient needs to know before undergoing this intervention:

To have a tight elastic belt that will have to be worn for 2-3 weeks after the intervention.

Not to take Aspirin 1 week before surgery.

To consult an anaesthesiologist before the intervention.

Blood tests.

Drainage – to be removed on the next day along with the elastic dressing.

The sutures are removed 10 days after the intervention.
